An evolutionary framework connecting ancient human biology to modern neuroinflammatory disease
Current work
In 2026 I set out the Lipid Encephalisation and Mismatch Hypothesis (LEMH), which proposes that lipid-driven dietary transitions across the hominin lineage shaped the APOE allele sequence, and that the modern removal of those ancestral conditions is relevant to neuroinflammatory and neurodegenerative risk in APOE4 carriers. It is deposited as a preprint on Zenodo (June 2026) and is in preparation for submission to Evolution, Medicine and Public Health. A preprint has not been peer-reviewed, and LEMH is advanced as a theoretical framework generating testable predictions rather than as an established finding.
Alongside it I have been developing a classification of clinical action — proximate and ultimate medicine — which extends Tinbergen's evolutionary framework from explanation to intervention. That manuscript is in preparation.
A PhD research programme is in development for 2027 entry. The proposed thesis is methodological: it asks how interventions that target evolutionary mismatch should be evaluated, given that conventional trial design assumes properties such interventions do not have. LEMH and APOE provide the worked case.
I am also completing a systematic review of APOE-stratified response to omega-3 supplementation as part of my MSc.
Scope
This research is theoretical and exploratory. It does not form the basis of my clinical services, and no service is offered on the basis of APOE genotype. The Neurolithic Diet described within LEMH is a hypothesis-derived theoretical concept, not a prescribable dietary intervention.
